Why it happens
Most hacks come in through outdated plugins and themes with known security holes, “nulled” (pirated) plugins with hidden backdoors, or weak and reused passwords. Automated bots scan thousands of sites a day looking for these.
Deleting the obvious malware isn’t enough. Hackers leave backdoors so they can get back in. A proper clean-up finds and removes those too.
